Basics of House Job
As most of you will already be aware that after completion of MBBS/BDS every medico has to go through one year of mandatory internship at a teaching hospital recognized by Pakistan Medical Commission to make yourself eligible for a Full Medical License to practise as an individual doctor.
It is important to mention here that to apply for Full Medical License also known as RMP, you must have to pass the National Licensing Exam. But for starting a house job you don’t need to pass NLE, instead just after completion of your MBBS/BDS degree you can start your house job straight away, no matter whether you are a local graduate or a foreign one.
You can also do a house job in a foreign country in any teaching hospital recognized by Pakistan Medical Commission.
Rotations of House Job
House Job structure in Pakistan is made by the National Medical and Dental Academic Board working under the umbrella of PMC. But regarding the structure of the house job, no new instructions have been issued by the board & the previous structure issued by Pakistan Medical & Dental Council in 2018 is considered as a baseline document for house job structure.
Basically, the house job is divided into four modules spread over the period of 3 months each, out of which two modules are compulsory.
House Job Structure for Medical Graduates
Let’s make it more clear & discuss each module individually, so you can have a clear idea about the house job structure.
☞ Module 1 General Medicine – The 1st module will consist of a period of three months & during this the house officers will work in General Medicine including emergencies and out patients’ facilities & this is one of two compulsory modules. So you must have to take it.
☞ Module 2 Allied Medicine – The 2nd module will also consist of a period of three months during which house officers can work in paediatrics, psychiatry, cardiology, nephrology & dermatology departments on the choice of their own. Ideally, you should not try to cover all these specialities, instead you should at least give 4 weeks each to three of the above specialities.
☞ Module 3 General Surgery – The 3rd module will again consist of a period of three months & during this the house officers will work in General Surgery including emergencies and out patients’ facilities & this is also a compulsory module.
☞ Module 4 Allied Surgery – The 4th & the last module also consist of a period of three months & during this the house officers work in Gynaecology and Obstetrics, Radiology, ENT & Eye.

➜ Each Compulsory module like Medicine & its allied module should be completed separately at the same hospital. However, one module of three months duration at specialized hospitals of allied subjects like paediatrics medicine or cardiology or nephrology shall be accepted.
➜ Experience gained at two different hospitals for sub-components of compulsory modules shall not be accepted.
➜ Experience gained for one module (three months) of General Surgery and another module (three months) of Allied Medicine subjects shall not be allowed. In other words, you have to go through modules in order. First you have to do one compulsory module & then you will have to do the allied of the same compulsory module.
House Job Structure for Dental Graduates
If you are a BDS graduate, then your house job structure will consist of the following four modules of 3 months duration each.
☞ Module 1 Prosthodontics and Allied – This module will be spread over the period of three months during which house officers will work in Prosthodontics and Allied.
☞ Module 2 Operative dentistry and Allied – This module will be spread over the period of three months during which house officers will work in Operative Dentistry and Allied.
☞ Module 3 Orthodontics and Allied – This module will be spread over the period of three months during which house officers will work in Orthodontics and Allied.
☞ Module 4 Oral and Maxillofacial Surgery and Allied – This module will be spread over the period of three months during which house officers will work in Oral and Maxillofacial Surgery and Allied.

A maximum of six months of house job or internship in one particular speciality may be allowed, the rest of the six months shall be utilized by rotations to cover all disciplines of dentistry.
House Job Salary in Pakistan
Regarding the house job salary, there is much confusion in medicos & of course it should be, as the salary is different in each province & is determined by the respective government. Each medical/dental college & teaching hospital has to follow that stipend policy as directed by PMC.
According to PMC orders regarding house job, It is the responsibility of every medical & dental college (both public & private) to provide their graduates with Paid house job in their respective affiliated teaching hospitals.
To make it clear PMC has issued special orders for private graduates that their pay should not be less than those being paid to government graduates at government hospitals in respective provinces/states.
So if any public hospital is paying a stipend of Rs 45,555 to their House Officer in Punjab, then it becomes mandatory for all the private medical & dental colleges to pay at least 45,555 PKR to their House Officers & so is the case for the rest of the provinces including Capital territory.
In addition, if any medical or dental college is unable to accommodate all of its graduates for house job in their teaching hospital, then their students may apply at another hospital for house job but still his/her stipend will be paid by his/her graduating college.
But if any graduate voluntarily wants to do house job in any other hospital due to whatever his/her personal reason, then his/her native college will not be responsible to pay the stipend.

Frequently Asked Questions About House Job
Can any private graduate get a paid House Job at a government hospital?
The decision to pay the private graduate at government hospitals is in the hands of the provincial governments & unfortunately the governments of all the four provinces have strictly ordered Not to Pay Stipend to any private or foreign graduate even if paid slots are empty in any government hospital.
Those empty paid slots will be kept dormant but will not be given to any private graduate. However, these paid slots can be awarded to public graduates of any other college of the province or even to public graduates of any other province of the country.

The only exception is Capital Territory hospitals in Islamabad like PIMS & Polyclinic where the federal government has allowed to grant paid house job to private graduates also. But in this case also, the priority will be given to public graduates.
Can any government graduate get a paid House Job at a government hospital other than his/her native college hospital?
As already mentioned, yes a public graduate may get a paid house job in any public hospital other than his native institute hospital but preference will be given to own graduates in respective hospitals but still any paid slot is empty, then he/she may get that.
Can any public/private graduate get a paid House Job at a private hospital?
It is the sole power of the private hospital to decide whether to give or not, a house job stipend to external graduates doing house jobs in their hospital & in most of the cases they do not pay the stipend because as per government rules it is not their responsibility. But if any private hospital is rich enough, they may decide to pay & this case is rare.
Again it is important to mention here that the above rule is for graduates of any external college & for its own graduates, it is the responsibility of the hospital to pay them.
